Care Coordinator (12 month Maternity Leave Contract)

We currently have an opportunity for an experienced and passionate Care Coordinator to join the fantastic team at our Glenrose Court site on a 12 month maternity leave contract.

 

The Position:

The Care Coordinator  will work as part of the multi-disciplinary team to support each individual's holistic care needs and ensure the assessment, implementation and regular review of each person's care plan. As part of our holistic model of service delivery, this role will ensure positive resident focused care outcomes are met and maintained.

 

The Care Coordinator will actively promote opportunities for clients to engage in the daily activities that provide them with meaning and purpose, and will lead the team in fostering a genuinely holistic and individualised approach to overall care and support.

 

The Care Coordinator is responsible for:

  • Clinical leadership and support to the Registered Nurses, Enrolled Nurses and Care staff
  • Liaising with residents and family members to support each individual's needs
  • Ensuring individual assessments are completed in a timely manner
  • Ensuring robust documentation is maintained to support ACFI submissions
  • Timely reporting of monthly statistics
  • First line staff management and support

The Person:

The person we are seeking will be a highly competent Registered Nurse with a minimum of 3 years experience in a similar position within the Residential Aged Care environment.

 

You will be an outstanding communicator with highly effective interpersonal and relationship management skills. Your ability to demonstrate clinical leadership and capacity to engage with people in a  sensitive and empathetic manner will enable your ready transition to your new role.

 

You will possess  excellent time management and prioritisation skills and  demonstrate  sound understanding of ACFI.  

 

You will be committed to continuous quality review and improvement and have a sound knowledge of various legislative responsibilities including the new Aged Care Standards and Work Health & Safety. 

 

 Key experience skills & attributes:

  • Registered Nurse with no restrictions to registration
  • Sound working knowledge of ACFI
  • Demonstrated ability to manage a team and develop a cooperative environment
  • Excellent time management with the flexibility to meet changing requirements
  • Excellent customer service delivery
  • Excellent communication and people skills
  • Demonstrated understanding and compliance with Aged Care Standards and WHS legislation
